Athletic Forms

In order to participate each player should have all required documents submitted to the JHS athletic office by August 1st .  Please note that all required forms (except physical) are submitted electronically via the RankOne System.  

 

Please note that downloadable forms are also available for home educated students. They should be submitted directly to the JHS Athletic Office. Lindsey Petrick may be reached at lpetrick@everettsd.org
